SmartHF Study is a 12-week clinical test to see if a special web app helps improve medicine use for people with heart failure. Heart failure with reduced ejection fraction (HFrEF) is when the heart doesn't pump as well as it should. The study will compare two groups: one uses the app, and the other does not. The app, called SmartHF, looks at electronic health records (EHR) and suggests better ways to use medications. These suggestions are easy to understand, and patients can share them with their doctor.

Eligibility: You can join if you are 18 or older, have heart failure with a specific heart condition (LVEF ≤ 40%), have internet access, and meet some health conditions.
