The goal of this study is to check if the Glycemic Excursion Minimization (**GEM**) plan helps Black adults with type 2 diabetes. The GEM plan includes coaching people to eat fewer carbohydrates and to be more active after meals. A continuous glucose monitor (*CGM*), a small device that checks blood sugar levels all the time, will give feedback. This might help improve blood sugar, reduce stress from diabetes, and boost confidence in managing the disease.
This study will see if the GEM plan can be used and is effective for adults with type 2 diabetes, who do not use insulin or secretagogues (medicines that help the body release insulin).
- Participants must have type 2 diabetes and be able to change diet and exercise habits.
- Participants should not be on insulin, secretagogues, or medications that make it hard to lose weight, like prednisone. Pregnant women or those planning to become pregnant soon cannot join.
- No history of serious low blood sugar (hypoglycemia).
