Study Summary: This study aims to help people manage diabetes better at the Midtown General Internal Medicine Clinic. Diabetes is a condition where your blood sugar levels are too high. The clinic will have special days with doctors who focus only on diabetes care. They will also help with other parts of life that affect diabetes, like food, housing, exercise, and mental health. The study will also teach medical residents, who are doctors in training, about diabetes care.

Many Black and Hispanic people have more problems from diabetes than white people. This study wants to change that by using a team of different experts, like doctors and dietitians, working together. They will help patients in one place and use new tools like Continuous Glucose Monitoring (CGM) to track blood sugar levels better.

  • Length of Study: Ongoing with special clinic days.
  • Eligibility: Patients aged 18+ with HbA1c (a measure of blood sugar) of 9% or higher and medical residents.
  • Exclusions: Patients not following up at Midtown, pregnant women, or those with an endocrinology specialist.
