Testing an Integrated Care Model Trial

This study is testing a plan to help adults with type 1 diabetes (T1D), a condition where the body can't make insulin, manage their health better. For this research, adults who have T1D will share information about their mental and physical health through online surveys every six months over a period of 18 months. Some people will also get extra help from a Behavioral Health Consultant (BHC), who is someone trained to help with health and stress. The BHC will talk with them over the phone or on Zoom, helping them set goals and find ways to stay healthy. These sessions will last between 30 to 60 minutes, and there will be 2-3 sessions in the first year. The study will take place at the Grady Diabetes Clinic. People who do not speak English or do not have a phone will not be able to join. Also, people with severe mental health issues will be referred to specialists instead.

  • Study Length: 18 months with surveys every 6 months.
  • Visits: 2-3 phone/Zoom sessions with a consultant in the first year.
  • Exclusion: Non-English speakers or no phone access cannot participate.
