This study is for people hospitalized due to infections from injecting opioids. It's testing two ways to help: Integrated Clinic (IC) and NavSTAR (NS). The IC combines care for infections and opioid use disorder (OUD) in one clinic, either in person or via telemedicine, with monthly appointments for 6 months. The NS provides support from a navigation team to connect participants with outpatient treatment and medical care for 6 months after leaving the hospital. The study will check if these help reduce hospital readmissions and improve health. Participants will be randomly assigned to one of these groups or get usual care (TAU). Researchers will follow participants for 12 months. To join, you must be 18 years or older, hospitalized due to a qualifying infection from injecting opioids. Some infections include bone infections, abscesses, or heart infections. You can't join if your infection isn't from drug use, can't consent, or are in hospice or incarcerated.
